Who Can Apply:
- A US citizen or a legal and permanent resident of the US
- Diagnosed with AD, AS, CD, HS, JIA, Ps, PsA, RA, UC, or UV by a healthcare professional. (If the applicant has been diagnosed with more than one of these inflammatory diseases, the applicant should choose the most relevant inflammatory disease for this application.)
- Current high school seniors or graduates or current postsecondary undergradtuates or graduate level students who plan to enroll in full-time undergraduate or graduate study at an accredited two-or-four-year college, university or vocational-technical school for the entire 2026-2027 academic year.
- Pursuing a Certificate or an Associate’s, Bachelor’s, Master’s or PhD Degree.
Submission:
- A current, complete transcript of grades. Grade reports are not accepted. Transcripts must display:
- Student name
- School name
- Grades
- Credit hours for each course and term in which each course was taken
